Online registration
We know your time is valuable. That’s why we offer convenient online registration for services such as surgical procedures, mammograms, and diagnostic tests and treatments.

Participating insurance plans
We accept all forms of government insurance from the U.S., Canada and other countries. We also accept Medicare, Medicaid, Civilian Health and Medical Program of the Uniformed Services (CHAMPUS), and workers compensation.
Advance care planning
Advance directives are legal documents that allow you to give directions for your future medical care. Advance directives help you protect your right to request or refuse care.
Patient rights and responsibilities
We respect the dignity and pride of each individual we serve. We want our patients to be informed of their rights, responsibilities, and have access to contacts for complaints and grievances.
Parkridge Health System welcomes you as a patient. Our admitting areas are typically located on the first floor of our facilities, just inside the main entrances. They are open for registration 24 hours a day, 7 days a week. We are happy to answer any questions you might have concerning your visit. To obtain an appointment for outpatient services, please call Central Scheduling at (423) 493-1271. A member of our team will work with you and your physician to arrange your appointment at a time convenient to your schedule.
Registration
We want your registration process to go as smoothly as possible. That is why we have created a guide to help ease the process.
You may be required to sign the following:
- Consent to treatment
- Authorization to bill insurance
If you are a Medicare recipient, you will be required to sign:
- Medicare Secondary questionnaire
- Medicare Rights Statement
Remember to bring with you at the time of registration the following items:
- Social Security Card
- All active insurance cards
- Picture ID
- Living Will, if you have one
- Durable Power of Attorney, if you have one
